Scrubbing Files: How to Resolve the "Unable to Import the Scrubbing File" Error

Overview

When importing a scrubbing file into OneSpot, you may encounter the following error: 

"Unable to import the Scrubbing file for the following locations <<Enter Locations HERE> Reason: Each invoice line item(s) may not contain more than 4 diagnosis codes. Review claim ID(s) XXXXXX in RevolutionEHR Please correct and try again."

This error occurs when at least one claim in RevolutionEHR contains more than four diagnosis codes on a single line item. RevolutionEHR enforces a strict limit of four diagnosis codes per line item when generating 837 electronic claim files.

Any claim that exceeds this limit cannot be exported from RevolutionEHR or imported into OneSpot until it is corrected. The error message displayed in OneSpot identifies the affected location(s) and claim number(s), allowing you to quickly locate and update the impacted claims in RevolutionEHR using the instructions outlined below.


How to Fix the Error

Follow these steps to identify and correct the claim(s):

1. Access Claims: Login to RevolutionEHR and navigate to Accounting > Claims

2. Select Advanced Search.

3. Set the following search filters: 

  • Location: Select the X next to the location field to include all locations.
  • Claim ID: Copy and paste the claim ID from the OneSpot error message.
  • Status: Set to Active
  • Submit Status: Set to Ready to Submit.
  • Submit Method: Set to Electronic.

4. Select Search

5. The affected claim will appear in the search results. Review the claim and remove any diagnosis codes exceeding the four-code limit from the affected line item. Repeat steps 3–5 for each additional Claim ID listed in the error message.

Note: If the claim does not appear in the search results, verify in RevolutionEHR that your user account has access to the location associated with the claim (as listed in the OneSpot error message).

6. Import to OneSpot: Login to OneSpot, navigate to Scrubbing, then select Import Claims.
